PADI Open Water Diver Certification
A student diver can present the PADI Universal referral for Open Water Diver certification to a instructor. It states that the diver took initial training from another diver training organization. The form directs the instructor of diving to complete the student’s open-water dives and sign a declaration. This statement should be sent to the original instructor so that they can certify the diver.
The Universal referral form can be used to get certification. But, PADI demands that you complete the course within 12 month of your last training. This is usually a two- or three day course. You must be able and willing to dive with your buddy up to 60 feet/18 metres.

Medical statement
To get certified as a diver, you need to complete the Medical Statement to Universal Referral Form. This is an important document to ensure that you are healthy before diving. This document will ensure that you are safe underwater. Make sure to answer each question honestly.
First, get your physician to sign the form. This is a requirement when you first start training as a scuba diver. If you have a new condition, a medical release will be required. Last but not least, make sure you have a photocopy to your certification card.
Logbook
Before a diver can dive open water, he or she must complete a medical statement. This will usually be completed before the certification course begins. A doctor must sign the medical release. You will need a new release if your medical situation changes. A medical release is available through PADI. The PADI Release Of Liability/Assumption Risk/Non Agency Acknowledgement Form-General Training or the EU version is equivalent to the medical release.
The universal referral forms are different from those used in a dive logbook. It will list the open water dives and knowledge-development sections the student has completed. This document also includes an instructor signed certification that the dives were completed. The student is then required to take the form back to their instructor to be certified as a diver.

Course completion requirements
The Universal Referral Program covers most well-respected scuba certification bodies. The Universal Referral Program means that a diver may take the theory or pool portion of an open water course with their local dive center and then do the open water check out dives with another agency. This is done with the cooperation of a dive center with a universal referral program such as SSI, NAUI, NASDS, YMCA, or other organizations.
PADI Open Water Divers requires that students pass the Knowledge Development as well as Open Water Dives sections. A medical release must be signed by the student diver before he or she can dive. To accompany their certification card, the student diver must also submit a photograph. Digital images are acceptable.
